                             Requirements

 

First Issue (Fresh)

 

Original and photocopies of:

  • Duly completed passport application form and 2 passport sized photographs.

  • Payment acknowledgement slip/receipt.

  • Guarantor's Form duly completed and signed by the Guarantor with the copy of his/her Nigeria passport data page and 2 passport photographs attached.

  • Birth Certificate or Age Declaration or Attestation of Birth (as applicable) from National Population Commission duly legalized by the Ministry of foreign affairs Abuja (For applicants born in Nigeria).

  • Certificate of Origin or Certificate of Identification from the applicant's LGA in Nigeria or National ID card.

  • Copy of any other passport in possession.

  • Resident Permit (as applicable).

  • Residents' registration document (ID Card').

 

Additional requirements for children under 18 years

  • Letter of consent written and signed by both biological parents (whose names must appear on the birth certificate).

  • Evidence of Nigerian citizenship of parent(s).

  • Parents' passport data pages.

  • Single parents must present evidence of total custody ( divorce court judgement or death certificate of the other parent).

 

Reissue (Renewal)

 

Original and photocopies of:

  • Expired/expiring passport booklet.

  • Duly completed passport application form and 2 passport sized photographs.

  • Payment acknowledgment slip/receipt.

  • Guarantor's Form duly completed and signed by the Guarantor with the copy of his/her Nigeria passport data page and 2 passport photographs attached.

  • Resident Permit  (as applicable).

  • Residents’ registration document (ID Card').

 

Additional requirements for children under 18 years

  • Letter of consent written and signed by both biological parents (whose names must appear on the birth certificate).

  • Parents’ passport data pages.

  • Single parents must present evidence of total custody ( divorce court judgement or death certificate of the other parent).

 

Lost/Stolen/Damaged passport

 

Original and photocopies of:

  • Formally written and signed application for reissue of lost/stolen/damaged passport.

  • Damaged passport booklet/copy of lost or stolen passport´s data page showing passport number and date of issue, OR

  • Police report on the loss/stolen of passportExpired/expiring passport booklet. (must be translated in english and bring along the both versions)

  • Duly completed passport application form and 2 passport sized photographs.

  • Payment acknowledgement slip/receipt.

  • Guarantor's Form duly completed and signed by the Guarantor with the copy of his/her Nigeria passport data page and 2 passport photographs attached.

  • Resident Permit  (as applicable).

  • Residents’ registration document (ID card).

 

Additional requirements for children under 18 years

Change of Surname (due to marriage, divorce or death of spouse)

 

Original and photocopies of:

  • Formally written and signed application for reissue of passport with change of data as applicable.

  • Expired/expiring passport booklet.

  • Duly completed passport application form and 2 passport sized photographs.

  • Payment acknowledgement slip/receipt.

  • Guarantor's Form duly completed and signed by the Guarantor with the copy of his/her Nigeria passport data page and 2 passport photographs attached.

  • Resident Permit or foreign passport

  • Copy of residents’ registration document (ID Card). 

  • Marriage certificate.

  • Statutory evidence of change of name due to marital status (Affidavit etc.)

  • Legalized Publication of name change in any Nigerian newspaper.

  • Evidence of maiden name.

  • Spouse's proof of ID such as International Passport or National Identity Card.

  • Divorce certificate (for divorcees only)

  • Death Certificate of spouse (for widows only)

- Nigerian Embassy Passport Appointment  -

(1.0) New account Registration:
If you are new, you must sign up to create your appointment reservation. Fill the form and click submit. You will be sent a confirmation email. Check your incoming mail or the spam. Open the mail and click the included link to confirm your email address. The fields marked with red circle are required.

(1.1) Login registered users.
Login requires the email or the username, password used during the registration. User will not be able to login if he has not yet confirmed the email verification request sent to his email address after registration.

(2.1) Appointment Reservation Booking:
Appointment booking is based on a single date without time consideration. This means that you should not modify the TIME and the End Date / TIME.. Just select the Start Date you wish. Leave blank the "Title and the Description" fields. The payment ID and the Reference ID you received after Passport payment on the Immigration Website are required. Then Click "Create", your appointment will be created and approved authomatically. You will also receive a confirmation email. Print out or download on your smartphone the confirmation email because it's needed on the day of your appointment.

(2.2) Passport Individual Appointment:
Select this option for individual appointment. The system allows single appointment per individual basing on user account (email). You can not book a second appointment if there is an already pending. To book another appointment you need to delete the pending. However you can change the date of an appointment to another available date.

(2.3) Passport Group Appointment :
Select this option for a group appointment, such as a family. The group head-person. can book a max of 5 entries. This option is allowed only for a compact group, ex a family (Father, Mother and the Children). With the same account the group head-person books for the rest. For each group member, add the name - surname and select the age. Add the payment ID,  the Reference number and create. Repeat the same process for the rest of the group members. You can not book a second appointment if there is an already pending, To book another appointment you need to delete the pending. However you can change the date of an appointment to another available date or substitute a member, .


(3.1) Available days:
- Monday
- Tuesday
- Wednesday

(3.2) Non Available days:
The days of the week not listed and the eventual Nigerian and Italian Public Holidays

(3.3) To check available dates, Click on the top menu "Schedule - > Bookings". The available dates appear in white

(3.4) While checking the available dates, the system also gives the possibility to see the number of appointments already present in each date. It's useful if you want to avoid congested dates. Booked appointments appear in blue color

-- Frequent Questions --
(1) CAN I BOOK FOR A FRIEND:
Answer: Yes you can help your friend to create a single individual appointment with his email account or yours. Remember that the same email can not be used again if there is a pending appointment.

(2) CAN I CHANGE MY APPOINTMENT DATE:
Answer: Yes you can change the date to another available date. You only need to change the date, update and the previous appointment reference remains the same.

(3) HOW CAN I AVOID CROWDED DATES:
Answer: before selecting a date it's possible to verify the number of people booked on that date. Select from the top menu Schedule->Bookings. On each date you will see the number of the pending appointments in blue color

(4) CAN I USE GROUP BOOKING
Answer : Yes, you can use group booking for a family group or for any compact group. Any mis-use will not be attended to.

(5) WHAT ARE PAYMENT ID AND PAYMENT REFERENCE N.
Answer: The two numbers are issued after making passport payment on the Nigerian Immigration Website. The numbers are required to proceed with the appointment booking.

(6) WHAT HAPPENS IF NO-SHOW
Answer: There is no penalty. You can still create a new appointment. It's not possible to modify a passed appointment. You don't need a new registration. Login with your previous registration credentials.

(7) CAN I CREATE A LONG PERIOD APPOINTMENT:
Answer: Yes, there is no limit to your date choice, provided is not marked closed.

(8) CAN I CONTACT THE PASSPORT STAFF:
Answer: The best way is to use the online customer support chat found on the down right side of the HOME PAGE. You can receive immediate assistance from the online support staff,. If the service is off-line, you drop a message with your email address and you will be reached as soon as possible.

(9) FAKE AGENTS AND FAKE APPOINTMENT:
Answer: No one is authorized to book appointment for a third person behind payment. You can verify your appointment directly from your dashboard. If the appointment was done by a friend and you don't have the account credentials, you can verify it by contacting the Passport staff from the online support chat on our Home Page.

(10) DO I NEED TO PRINT MY APPOINTMENT RECEIPT:
Answer: Yes, if it's possible, otherwise just show to the appointment staff the confirmation email or the reference number. In any case your reservation can also be traced with your name, surname or email
